HOW TO ORDER:

Ordering is ONLY done by EMAILING or FAXING your Filled-Out "LIVE FIELDS" ORDER FORM [Download Order Form] No Phone or Online Ordering.

Options:

1. Simply go to the Coach Tools Page when you are ready to order.

• Download the Order Form and fill it out.
  
• Then “SAVE AS” using your last name e.g.: McLean CoL Form
  
• Next attach the Order Form and email colbooks@cybercopyusa.com or
• Fax Order Form to (805) 715-1535

2. You can download the ORDER FORM onto your computer desktop and keep a copy in a desktop file.

• Fill out the Order Form.
• NEXT “SAVE AS” using your last name e.g. McLean CoL Form.
• Then attach the Order Form and email colbooks@cybercopyusa.com or
• Fax Order Form to (805) 715-1535

After you email or fax your form to CyberCopy, they will confirm receipt of your email. Usually within 24 hours of receiving your form, CyberCopy will call you to get your credit card number or payment and confirm all the details of your order.

Be sure to fill out on the bottom of the form:
- Best Time & Number to reach you during Pacific Time business hours______

• Invoice accounts / PO (Purchase Orders)
  
• Check or Money Order
